Building connections for your recovery.

Community Connect helps individuals who live with mental health or substance use disorders. The program helps participants find recovery support, employment, and help with previous criminal activity. 

Care Coordinators

• A Care Coordinator assists with short and long-term goals, barriers, and provides a source of connection and support.

Peer Support Specialists

• A Peer Support Specialist is a professional with similar lived experience who offers hope, support, and advocacy.

Eligibility

• 18+ years old.

• North Dakota resident.

• Mental health or substance use disorder diagnosis impacting the ability to retain housing, employment, parenting, physical health, and more.
