    Community Uplift Program was founded on the idea of inclusivity with emphasis on providing support, resources, and assistance in areas of mental health, recovery, housing stability, and more for anyone in the community. We currently offer care coordination through Community Connect, Free Through Recovery, 1915i, and 1915c.

    Our mission is to be an agency that provides recovery supports to anyone struggling with substance use and/or mental health and we operate as a proudly LGBTQ+ affirming space.

    Our vision is to be an LGBTQ+ affirming space for holistic wellbeing for the community to access person-centered resources and supporting them to be successful on their own terms.

Care Coordination

Care coordination plays a crucial role in helping individuals achieve their goals. Our approach incorporates the S.M.A.R.T. goal-setting framework, ensuring that objectives are Specific, Measurable, Achievable, Relevant, and Time-bound.

Our Care Coordinators provide personalized support across various areas, including housing, recovery, mental health, financial stability, and more. By adopting a case management style, we ensure that our programs align with the unique needs of each participant.

Creating comprehensive care plans is a key function of our service. Care Coordinators work closely with participants to develop and implement S.M.A.R.T. goals that foster engagement and facilitate the attainment of realistic aspirations. We emphasize a person-centered approach, believing that meeting individuals where they are in their journey is essential for effective coordination and support.

Certified Peer Support

Certified Peer Support Specialists are individuals providing assistance, encouragement, and understanding to participants based on shared experiences. Our Certified Peer Support Specialists have faced similar challenges to our participants, such as those related to mental health, addiction/recovery, and more. Peer support offers a safe space for open dialogue, emotional validation, and practical advice, fostering a sense of community and belonging. Through mutual understanding, peer support can empower individuals, reduce feelings of isolation, and promote personal growth and recovery. Certified Peer Support Specialists work with the Care Coordinators to create a well-rounded support system to help our participants achieve their goals.
